The following terms and conditions govern the Life Line Screening OneLife Membership Program and the benefits available thereunder (“Membership Program”) offered by Life Line Screening of America Ltd. (“Life Line”, “we”, “us”, “our”). BEFORE SUBSCRIBING TO THE MEMBERSHIP PROGRAM YOU ARE URGED TO READ THESE TERMS & CONDITIONS CAREFULLY. Description of Membership Program
1. As a OneLife member you are entitled to receive the following Membership Program annual benefits (“Benefits”) for your personal use:

4. Membership Term. As a OneLife member, you shall be entitled to all of the annual Benefits available commencing on the date of acceptance of your application for enrollment (the “Membership Term”), or if you enroll in a recurring membership, you shall be entitled to annual Benefits on the first anniversary of your OneLife membership.
5. Electronic Disclosure and Consent. By enrolling in the Membership Program, you hereby consent to receive all disclosures, notices and other communications in electronic form. You may request paper copies of required notices by calling 1.833.759.9022.
6. Membership Fees. The current membership fee (the “Membership Fee”) will automatically be billed to your designated credit or debit card on a monthly or annually recurring basis as authorized upon enrollment. If you use a debit card as the designated credit card account, you acknowledge that we will not be responsible for any fees or penalties associated with the failure of your debit card to be approved for the Membership Fee. Unless you cancel your OneLife membership by contacting us at 1.833.759.9022, we will automatically continue your OneLife membership each month or year as authorized upon enrollment by charging your designated credit or debit card.
7. Right to Cancel. You have the right to cancel your OneLife membership at any time by calling us at 1.833.759.9022. If you decide to cancel, the amount you owe is the balance for any screenings and blood work received but not yet offset by payment of the Membership Fee. Amounts due (“Balance Billed Services”), if any, will be billed to the same credit or debit card we have on file for you. You will also forfeit any unused annual Benefits which you otherwise would have received through the end of your Membership Term. Due to the number and nature of the services provided by the Membership Program (many of which are not “Balance Billed Services”) you understand and agree that refunds cannot be issued. If you have any issues or questions about Balance Billed Services, you should direct them to the phone number or address above.
8. Your Representations and Obligations. In return for the annual Benefits available under the Membership Program you make the following representations and acknowledge the following obligations to us: (a) you have read these “Membership Terms & Conditions” carefully and understand the provisions of the Membership Program and the billing method for the payment of the monthly or annual Membership Fee; (b) you may cancel your OneLife membership at any time and will not be entitled to a refund of the pro rata unused portion of the Membership Fee and you will forfeit any unused annual Benefits; (c) the Membership Term will continue automatically each month on the same day of the month you first enrolled until your OneLife membership is canceled; or in the case of annual memberships, the Membership Term will continue automatically each year on the same month and date you first enrolled, until your OneLife membership is canceled; (d) the Benefits are not assignable without our express written consent; and (e) you understand that the annual Benefits and the Membership Program are not health insurance.
9. Governing Law. Your OneLife membership, and each of our respective rights and obligations shall be governed by, and construed in accordance with, the laws of the State of Ohio. Nothing herein stated shall deprive you of the benefits of your state’s consumer protection laws.
10. Entire Agreement. These Membership terms & conditions contain the entire agreement between us and no other representations, inducements, promises or otherwise between us shall be of any force or effect.
